Filing Insurance
We participate in many insurance plans, including Medicare, Medicaid, and most HMOs and PPOs in the area. We will help you file with the insurer for you. However, you remain personally responsible for any portion of the bill not covered by insurance. It is your obligation to thoroughly understand the coverage and exceptions of your particular policy. Patients having HMO and PPO plans that require a referral number must contact their Primary Care Physician (PCP) for that number prior to your visit. The insurance companies will not allow our office to contact your PCP for you. You will be responsible for all charges if you fail to obtain a referral number.
Billing and Collection Policy
If you provided us with insurance information, your insurance company will be billed as a service to you. We will submit benefit claims to your insurance company if all required information and authorization is provided. You are responsible for deductibles, co-insurance, and any items not paid by insurance. You should contact your insurance company to determine your cost-sharing responsibilities, if any.
